SMS TERMS AND CONDITIONS Valley Forklift LLC — Internal SMS Notifications Effective date: October 30, 2026 Program Description Valley Forklift LLC provides automated SMS notifications to employees and designated recipients who opt in. Messages relate to employee form submissions, parts requests, and related internal work activity. Messages may include links to relevant records that require an authorized login. This program does not include marketing or promotional messages. Enrollment and Consent To enroll, provide your mobile phone number and select the optional SMS consent checkbox on our enrollment form. Enrollment is voluntary. Submitting a form without selecting the SMS consent checkbox does not enroll you. Consent is not a condition of purchase or of submitting other company forms. You must own the mobile number provided or be authorized to consent to messages sent to that number. Message Frequency and Charges Message frequency varies based on work activity and form submissions. Message and data rates may apply. Contact your wireless provider with questions about your plan or messaging charges. How to Unsubscribe Reply STOP to unsubscribe. You may receive one final message confirming that you have been unsubscribed. No further program messages will be sent unless you enroll again. Carriers are not liable for delayed or undelivered messages.
